QPS Award Eligibility

Eligibility Verification
Candidates must meet all eligibility criteria as listed below and verified by their manager(s).


QPS nomination which includes an individual clinician or if a collaboration: two clinicians OR a clinician and a UHG employee:

  • Must hold an active and unrestricted clinical license if nominating an individual and at least one of two collaborators if a collaboration nomination
  • May work in any role across the enterprise including those that do not require a clinical license
  • Must be regular, full or part-time employees in individual contributor or leadership role
  • All physician nominees must be employed by UHG/UHC/Optum (including non-integrated entities)
  • Non-physician Nominee can be up to and including grade 32, physicians up to and including M3 (varies for non-integrated entities, addressed on a case-by-case basis)
  • As of April 22, 2024, a minimum of 18 months of tenure with UnitedHealth Group, including service with an acquired and/or non-integrated entity, from the date when nominations open. Must have no current performance management issues and no written performance management actions during the previous 12 months
  • Must have manager support for nomination - confirmed when submitting
  • Both domestic and internation employeers are eligible
  • Cannot be a past Sages winner - individual or collaboration

Ineligible employees

  • Per diem
  • Clinicians retiring or leaving the organization on or before October 2024 are not eligible

Collaboration Award Eligibility

Collaboration nomination which includes two clinicians OR a clinician and a UHG employee:

  • At least one of the nominees must hold an active and unrestricted clinical license
  • May work in any roles across the enterprise including those that do not require a clinical license
  • Must be regular, full or part-time employees
  • All physician nominees must be employed by UHG/UHC/Optum (including non-integrated entities)
  • Non-physician Nominee can be up to and including grade 32, physicians up to and including M3 (varies for non-integrated entities, addressed on a case-by-case basis)
  • As of April 22, 2024, have a minimum of 18 months of tenure with UnitedHealth Group, including service with an acquired and/or non-integrated entity
  • Must have no current performance management issues and no written performance management actions during the previous 12 months
