Individual Award

This award recognizes a licensed clinician, working in any type of role at UHG, for their outstanding work. Nomination as an individual means their work is being recognized by a nominator(s) that feel their contributions should be celebrated.​​​​​​​ The Individual Award is the original Sages award, first introduced in 2004.

Nomination Information

If a candidate does not meet the eligibility requirements, the nomination should not be submitted. If the candidate meets eligibility requirements:

  • Work with your colleague(s) and or manager to write the nomination - add their names on the nomination site
  • Contact their manager to ensure they support the nomination
  • Go to the Sages nomination site when open Spring of 2024
  • You are encouraged to work on the nomination for any length of time prior to submission. Here is a template to use.

Sages serve our enterprise in a variety of roles and the specific examples that best describe their contribution to business success will vary from candidate to candidate, role to role and business to business.


Example of Past Winning Nomination

From the early 2000s while seeing members in the [business] to overseeing utilization for four key states in the Central Region, [name] has been an exemplary employee. [name] has always placed the member at the center of every activity, wondering what he can personally do to ensure a better experience for the member along their journey. He understood that notes needed to be completed with ease in order to get determinations of coverage returned and communicated faster to the members. He developed a tool for his local team that would help generate these notes. A tool that would go on to be integrated into a larger automation project, ensuring that turnaround times and licensure needs were met. He continues to work with project groups on developing and automating templates for the [business] to use that will interface with the current systems being utilized. While continuously innovating, as you would expect, [name] maintains high productivity and performance, always seeking ways to be more efficient in the services we provide.

[name] has built numerous relationships both internal and external. By always acknowledging the needs of the member, he relates to our providers and ensures that the members needs are being met. He keeps a keen eye on the social determinants of health which promotes the community tenure of our fragile population. He is aware of the shadow he casts and is always professional and respectful in his encounters. He loves to teach and never shies away from an opportunity to enlighten others. Without hesitation, [name] accepted the call to train all the [business] on the LOCUS/ECSII and CASII guidelines in which we were transitioning. He automated tools to help the Care Advocates score these guidelines as well as those of ASAM, for Substance Use Disorders. He continues to provide the new hire training on the guidelines as new employees join the organization. His innovations are estimated to have a savings of $1.5 million per year for the organization.

He starts and ends each day with compassion. He not only empathizes with what is going on around him, but he implements compassionate change in order to make a difference.
