Local partners know how best to connect their communities to care. That's why we're teaming up with organizations in communities like Wyandotte County, Kansas, to help people live healthier lives. By addressing health and socio-economic barriers with an on-the-ground approach, we can connect more individuals to better care.

Across Kansas, many families face significant barriers to better health, such as language, availability of physicians, affordability and transportation. According to United Health Foundation's America's Health Rankings, Kansas ranks 31st in number of primary care providers statewide. There are only 129 primary care providers per 100,000 people throughout the state.

By empowering local community health workers outside of Kansas City, the Community Health Council of Wyandotte County has provided culturally competent, direct care to more than 1,000 Kansans. That means 1,000 more personal connections and solutions delivered to those who need them most.

There is no one-size-fits-all solution for every community. But through local partnerships, we can address the distinct health needs within underserved Kansas communities, remove barriers and improve access to care for some of the state's most vulnerable residents. Through these innovative partnerships, we can help make the health system work better for all.
